The most misleading lab value on your bloodwork
Hey,
Let’s talk about one of the most misleading labs I see all the time: Vitamin B12.
The standard serum B12 test only tells us what’s floating in your blood at the moment. But your actual cellular B12 status—what your tissues can *use*—is often a very different story.
When your body starts running low, it can temporarily dump B12 into the bloodstream to compensate—making your lab result look “normal” or even “high,” while your cells are still starving for it.
And here’s the situation—there are a lot of reasons people can become B12 deficient, even with a decent diet.
You may be at risk if you:
- Eat mostly plant-based or vegetarian
- Have low stomach acid (common with age or stress)
- Take acid blockers or PPIs (like Nexium or Prilosec)
- Struggle with digestive issues or gut inflammation
- Have had H. pylori or other chronic GI infections
- Are missing cofactors like folate or B6
- Have MTHFR, COMT, or other methylation issues
Symptoms can show up as:
- Fatigue
- Brain fog
- Poor recovery
- Numbness or tingling
- Mood changes
